TGCW’s ACT program is an evidenced-based practice that offers treatment, rehabilitation, and support services, using a person-center, recovery-based approach, to individuals that have been diagnosed with serious mental illness (SMI). Typically, individuals severed by ACT have a serious and persistent psychiatric disorder and a treatment history that has been characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services.

ACT services are provided by a mobile, trans-disciplinary team who meet at the program participant’s residence and in mutually agreed upon settings in the community where the participant feels comfortable. ACT supports recovery through a highly individualized approach that provides participants with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The nature and intensity of ACT services are developed through the person-centered service planning process and adjusted through the process of daily team meetings.

The goal of ACT services is to assist individuals to achieve their personally meaningful goals and life roles. ACT integrates the principles to cultural competence, addressing the impact of discrimination/stigma, and inter-system collaboration into its services philosophy. An essential aspect of ACT is recognizing the importance of family, community-based, and faith-based supports.

PRIMARY FUNCTIONS:

  • Provide psycho-educational, rehabilitation, and recovery sessions either in one-to-one or group settings.

  • Provide individual counseling sessions using social work clinical modalities.

  • Conduct intakes as referred by the County, and complete psychosocial assessments.

  • Take responsibility for preparation and timely submission of all required documentation of assigned clients including progress notes, treatment plans and other assessments.

  • Participate in weekly treatment planning and clients' transitioning tasks.

  • Offer clinical modalities services to clients including the Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Relapse Prevention, Family Therapy, Crisis Intervention support, Solution-Focused Therapy, Task-Centered Practice, and other psychotherapy treatment modalities.

    ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:

  • Work as part of a team in providing care coordination and wrap-around services including needs assessment, wellness skill development, hospital appointments, vocational, and housing needs.

  • Additional assignments may be determined at a future date.

    QUALIFICATIONS AND ATTRIBUTES:

  • Previous experience in human services field is preferred.

  • Strong organization skills with accurate attention to detail.

  • Sufficient skills to communicate effectively with both clients and staff.

    EDUCATION AND EXPERIENCE:

  • Must be a current graduate level Social Work student.

    PHYSICAL CHARACTERISTICS:

  • Must be capable to sit or stand in front of a computer for moderate-periods of time

  • Able to work in open space floor plan

  • Must be capable to move throughout work day and follow individuals

  • Work alongside individuals within 3 feet

  • Occasional lifting of > _15+pounds
